Pilot error, fatigue and failure to follow safety guidelines were behind the crash of TransAsia flight 222 in July 2014, a report by Taiwan’s aviation authorities said on Friday. The plane crashed during a heavy storm near Magong Airport on Penghu Island, killing 48 people onboard. Ten survived.
